Have played a LOT of campaign games, including many that would be considered "Monsters".  More than a couple have gone to the very last turn, simply because the players were committed to doing so and not because the game was in doubt.  I've also played in many where one side surrenders because the game was out of reach and therefore no fun for one side.  Both are acceptable conclusions to me.  

In regards to Bolt Out of the Blue specifically I have never played it, considered it recently, as well as Certain Strike, but we opted instead for the Gotterdammerung Scenario instead and are glad we did.  That one ended in a mutually agreed upon tie (we optioned for Nuclear Termination Optional Rule so went into it knowing that was the likely outcome, and in my opinion all of the Europe based MC campaigns should be played that way).
