Doc, you do not list your country of origin, so I do not know if it is a language thing or if you think we know what your are talking about and posted your message in that framework.
What "survey" are you talking about and what does "extra achievements" mean? :chin:

If you're talking about starting a "poll", when you start a new thread, right below the text box area, and above the submit button is an area for starting a poll. How many "options" means how many answers to your poll question would you like to add? The default is two. Click yes you'd like to start a poll in that area and hit the submit button, just like you always do. The next screen will give you the areas for adding your options/questions.

Ahhhh. Ha! You'll find that under your Control Panel link, very top right of the site. On the next page, click on the blue "Blitz" link—last button, and the last popout option is the survey form.
