I occasionally place a unit directly next to a hex that has an enemy unit in it, but do not see that unit until it fires.

Sometimes I try to enter what is visually an empty hex only to discover an enemy unit there.

Are all units assigned a "never fire" command hidden until they fire or until an enemy attempts to enter that hex?

No.

Hidden units have what ever OPP fire settings you put on them, just like visible units.

If you want to be really nasty to your foe do this:

In a hex (buildings, woods are ideal) that has mutliple hidden units have one unit (engineers always are fun to do this with) with all its Opp fire settings off.

The unit may not be exposed when your enemy gets next to it. So your foe winds up disrupting the visible units but gets cruched when he goes to assault as the undisrupted hidden unit suddenly appears!

They are hidden until the start of your next turn, whereupon all our units within line of sight do a spotting roll to see if it becomes visible.

Exceptions are firing or moving, both of which reveal the hidden unit instantly.
